WAIVER AND RELEASE

At all classes, seminars, practice sessions, demonstrations and contests conducted by or SPARKS and sponsoring facility, special care is taken to prevent serious injuries and to minimize accidents.  MEMBER (or parent/guardian) states that He/She is fully aware that training in the martial arts of Tae Kwon Do, Karate, Kung Fu, Aikido, Judo, Hapkido, jujitsu, Boxing, Wrestling, Grappling, Krav Maga, MMA, self-defense, weapons and the like, even under the safest conditions, may be dangerous and MEMBER hereby knowingly and voluntarily agrees to accept full responsibility and assume all risk of injury, death and/or loss to His/Her person and/or property during SPARKS activities.  MEMBER agrees to obey the rules and regulations of SPARKS or any of its instructors put into effect to minimize these risks and agrees to apply due caution, care and common sense in practice to avoid injury to Himself/Herself or others. Member acknowledges that he/she maintains health insurance to cover any injury resulting from his/her participation in Martial Arts.

MEMBER knowingly, voluntarily and irrevocably waives any and all past, present and/or future claims, demands and causes of action which MEMBER now has or may have in the future against all officers, employees, agents, instructors, guest instructors, assistant instructors and/or students, while on the premises of sponsoring facility, or any other inside/outside facility, as a student, spectator, contestant and/or visitor, or in any other manner or form taking part in the exercise, practice and/or demonstration or said martial arts.  MEMBER certifies that He/She is physically sound enough to participate in martial arts program despite the rigors and dangers inherent in such an undertaking.  MEMBER accepts full responsibility for any injury, death, and/or loss to His/Her person or property incurred during a class or event at sponsoring facility or any other inside/outside facility.
